How to safely stop oral minoxidil (low dose)
Oral minoxidil is prescribed off-label for hair loss in the UK and has a clear set of reasons patients consider stopping. The cardiovascular profile means there is a particular care pattern at discontinuation.
Common reasons patients consider stopping
- • Facial or body hypertrichosis (unwanted hair elsewhere)
- • Ankle oedema or fluid retention

The pharmacology of stopping
Oral minoxidil for hair loss is used at much lower doses (0.625 to 5 mg) than the antihypertensive dose. Cardiovascular adverse effects, when present, are dose-related and typically resolve on stopping.

Missed doses, breaks and stopping: your questions
Can I skip oral minoxidil for a day?
Do not take an extra tablet to make up for it. The UK patient leaflet for Loniten, the 2.5 mg minoxidil tablet we dispense, opens its missed-dose advice with "Do not worry": miss out the forgotten dose completely, take the next dose at the normal time, and do not take a double dose. The cardiovascular side effects of oral minoxidil, such as ankle swelling and feeling dizzy on standing, are dose-related, which is why doubling up is the thing to avoid.

Can I skip oral minoxidil for a week?
The leaflet covers a forgotten dose, not a planned break, so there is no official guidance on what a week off does to your hair. Oral minoxidil has a half-life of about 4 hours and its effect is gone within days of the last tablet, so a week off is a week without treatment rather than a pause. Scalp hair gained on treatment is lost over 3 to 6 months after stopping. If you have missed several days, do not take extra tablets to catch up, and contact your prescriber before restarting so they can confirm your dose, particularly if you had palpitations, breathlessness, dizziness or swelling before the break.

What happens if you stop taking oral minoxidil?
The drug clears within days. The hair it helped you keep or grow goes more slowly: scalp hair gained through the longer growth phase is lost over the next 3 to 6 months as the follicles return to their untreated cycle, and hair loss resumes along its underlying pattern. Unwanted facial or body hair stimulated by treatment settles over weeks to months, as those hairs complete their growth cycle. For most people on the low doses used for hair loss, stopping in one step is appropriate, but the Loniten leaflet says not to stop or change your dose without seeing your doctor first, so speak to your prescriber before you stop.

Are there minoxidil side effects after stopping?
The cardiovascular side effects of oral minoxidil, such as ankle swelling or feeling dizzy on standing, are dose-related and typically resolve on stopping. Unwanted facial or body hair takes longer, weeks to months, because the hairs already stimulated have to finish their growth cycle. The most noticeable change after stopping is usually shedding of scalp hair gained on treatment, over 3 to 6 months: that is the treatment effect wearing off, not a new side effect. Palpitations, breathlessness or new dizziness on standing need prompt clinical review, whether or not you have already stopped. You can report any suspected side effect through the MHRA Yellow Card scheme.

How do I stop minoxidil without losing hair?
Honestly, hair that depends on minoxidil is not kept once minoxidil stops, so the practical question is whether you need to stop completely. If a side effect is the reason, a lower dose may resolve it without stopping, particularly mild ankle swelling or unwanted body hair. Some patients move back to topical minoxidil instead. If you do stop, photograph your scalp in the same lighting and at the same angle at 3, 6 and 12 months, so any decision to restart or change treatment is based on what has actually happened.
